Part of an Intensive Case Management Team that provided community-based support to participants connected to a Housing First Program. Assisted clients to achieve sustainable independence and tenancy. I provided client-centered care, landlord support and connection to services. ~~I was responsible for meeting one-on-one with my active caseload on a regular basis through home and community-based visits. I worked with clients to develop a strength-based and personalized service plan, grounded in the principles of Housing First and harm reduction, and monitored client progress towards goals. I provided life skills coaching, focusing on budgeting, nutrition, employment, creative problem solving, and other areas as identified by the client.I maintained working knowledge and collaborative relationships with relevant community supports, in order to connect clients with these resources. I attended appointments with clients, providing referrals and advocating for them as needed. I garnered first-hand experience working with clients at risk and an understanding of barriers related to mental health, addiction, trauma and poverty. This included working with Indigenous populations, and providing support with consideration of of culture, history, and current issues.
