Care Management Department Supervisor and Project Manager
-
Jan 2017 - Jan 2018
I was the first leader in a new department focused on establishing a value based culture in a company that was primarily fee for service. I managed and grew several projects (which later turned into sub-departments): a) Centralized Triage b) Central Care Management Scheduling c) Transitional Care Management (TCM)d) Centralized pre-visit data collection, documentation, and care navigation e) Other care models or payment models for private insurers (Cigna, UHC, BCBS) and f) CMS Innovation Initiatives (i.e. Oncology Care Model)g) Emergency department (ED) Diversion with select hospitalsI assessed, managed, and recruited talent for the projects/departments listed above. I later promoted and managed a lead for each department. I maintained oversight over each sub-department and analyzed performance data. I collaborated with senior leadership to create key performance indicators that were focused on maintaining quality and driving efficient, productive performance.I collaborated with and educated ~ 200+ providers and nearly 100 office managers on any new value based care/payment models and the associated requirements for accurate billingI provided cost benefit analyses to executive management on new programs or processes I planned on rolling out.I collaborated with HIM and IT professionals to improve communication processes within our electronic health record (EHR), which significantly reduced the amount of errors and the amount of time a patient is waiting to hear from his/her case/care manager.Notable Achievements:1. Expanded transitional care management (TCM) company-wide resulting in monthly revenues in-excess of $100k per month2. Increased accuracy of provider documentation compliance by 20% each quarter, thereby reducing billing errors and improving rate of collection3. Streamlined processes, which resulted in patients getting a phone call with their case/care manager 75% sooner and improving outcomes
