My current clinical experience is highly skilled specifically in the nutritional care of nutritionally vulnerable including those with multiple long term conditions including frailty, diabetes, dysphagia, pressure ulcers, those lacking mental capacity and in the last 12 months of their lives which requires working collaboratively within best interests. I was Champion lead on Mental Capacity , Adult Safeguarding for Dietetics Services colleagues and led the report writing of the annual dietetic documentation audit. Developed the SAFER under nutrition pathway, 'Food for Life' training, was the Dietetic representative and core member of the Leeds City Council Public Health Older People’s Food Matters Forum (LCCPH OPFMF) and led the engagement of LCH citywide nutritional champions. I also delivered effective training to a range of clients together with implementing an award-winning series of hydration and nutrition tools. Awarded a National Hydration Award in 2012 with LCCOPFMF in recognition of working with others in this projects 'remarkable yet relatively low cost campaign aimed at improving health whilst reducing treatment costs', worked with the local agenda on improving access to food and fluid and stage of change which led to more effective guidance in linking access to food and fluid into preventative pathways for malnutrition within a community perspective.
