In this position the focus is to improve patient safety by drawing on advanced research skills; to assist in the development of evidence-based practice to facilitate optimal patient outcomes. Examples include: The initiation of a Patient Safety Culture Survey since 2014 disseminated electronically and in hard copy to approximately 13000 MNHHS staff across 5 hospitals and 2 health services. The project involved adapting a survey instrument to meet the demographic demands of MNHHS, comprehensive collaboration with multiple healthcare teams and ancillary staff, data analysis, preparation of Executive reports and development of practical recommendations to improve patient safety culture. Results from the survey led to the introduction of Patient Safety Walk-arounds to improve management support for patient safety; raise the voice of staff and increase the visibility of Executives at the ward/unit level; initiate feedback loops to support the actioning of outcomes and communicate the results of the Walk-arounds to Executive committees.I progressed the development and implementation of a MNHHS Consumer Compliments and Feedback brochure in accordance with National Consumer Engagement requirements and governance expectations. I supported the progress of Safety and Quality agendas to align with Australian Commission on Healthcare Standards accreditation and periodic reviews improve patient safety and patient outcomes.

A document analysis was conducted to investigate the function of Queensland Health's Root Cause Analysis (RCA) to preventable death and permanent harm events in Queensland Health hospitals from 2009, 2010 and 2011 to improve patient safety. The RCA is a technique used to investigate harm events in healthcare that draws on organisational system models, human factor principles and causal factors to mitigate harm and improve patient safety.
