    This unique program between Hospice Care Inc and Winchester Home Care assisted patients and their families to access Hospice services sooner. In this role I was required to work both under Home Health regulations when caring for Winchester Home Care Patients and Hospice regulations under Hospice Care Inc. The program was truly challenging as I assisting patients and families to navigate the difficult waters of transitioning into end of life care. These challenges were physical, emotional and spiritual or a combination of all three. Responsibilities including managing a mixed caseload, symptom management, documentation and communication.

    As a Nurse Case Manager I was responsible for managing and providing care to a caseload of patients and their families who were residing in their own homes. The majority of my patients lived in downtown Boston and therefore had unique inner-city challenges to overcome in addition to their dying needs. As part of a team I was responsible for communicating and coordinating my patient’s care and responsibilities included pain and symptom management, triaging needs, planning a schedule, and being on call to visit and manage patient’s symptoms remotely on off hours. During that time I also assumed a senior nurse role which included being a preceptor to new employees and establishing a fun committee for improved morale and staff support.
