Lead team of quality nurses to support 46 practices throughout Maryland in achieving chronic disease management improvement goals and value-based incentives, and in meeting regulatory requirements.• Quality Assurance/Improvement Officer for LifeBridge Health Accountable Care Organization. Led ACO strategy to successfully achieve its quality reporting and improvement goals for its 20,000 Medicare beneficiaries. In its first three years, the ACO has improved its quality scores and retained shared savings in each year for total of over $21 million.• Achieved Exceptional Performance for MIPS incentive program.• Developed a community partnership with HealthCare Access Maryland that secured state funding for a three-year, $800,000 program for our hospital to provide intensive care coordination and reduce frequent users of the Emergency Department. Led the program design and managed its launch and evolution. Care Coordinators work intensively with frequent users to address underlying social, behavioral, and medical issues leading to repeat utilization. To date, the program has reduced visits by 26% for its high-risk client segment and avoided over $1.1 million in charges by connecting patients to more sustainable care and community resources. The program received the Maryland Patient Safety Center’s 2016 “Circle of Honor Award.”• Led implementation of the NCQA Patient-Centered Medical Home (PCMH) care delivery model for a primary care center serving Sinai hospital’s economically depressed community. Collaborated with physician, administrative, and community stakeholders to develop policies, infrastructure, and processes to enhance community health.
