The Better at Home Initiative was to plan, develop and support the implementation of South East Metro Health Service Partnership (SEMHSP) reform initiatives to drive improved safety and well-being standards for providing care in the home. This was a Victorian Health Department government-funded Project and included Alfred Health, Peninsula Health, and Monash Health. The role requirements were to:• Identify and prioritize healthcare projects to enhance home care. • Develop a project plan, monitor progress, and evaluate priority projects• Collaborate with primary, secondary care, and specialty services including Allied Health. • Assign responsibility, make recommendations, and provide progress reports to the SEMHSP Better at Home Initiative Steering Committee. • Monitor clinical consistency, safe, and timely service provision during testing phases and project implementation.• Develop and implement a rigorous evaluation framework, including sharing progress results and escalating risks to the Steering Committee
