    Assisted uninsured & underinsured patients in finding proper care post-treatment as well as improving their general health literacy. Successfully delivered medical care and information needed for a smooth transition between patients' journey of leaving the hospital and managing their own care at home. Primarily worked alongside patients with diabetes, chronic heart failure, COPD, cancer, and sepsis. Large majority of patients battled with homelessness, drug usage, or immigration/legal status. Followed throughout patients' journey weekly in order to fill Dignity Health's mission of Human Kindness. Streamlined Transitional Care's resources, worked throughout different departments of the hospital to deliver efficient care to patients, and represented volunteer and transitional care services during weekend events such as D-Backs Race Against Cancer.
