Capitol County Children’s Collaborative (CCCC) is a non-profit Care Management Organization (CMO) located in Mercer County, NJ. CCCC works with youth and their families ages 5-21 with emotional, behavioral and/or substance abuse challenges, intellectual, developmental, and/or intellectual disabilities. Capitol County Children's Collaborative 's Mission and Vision is to empower children and families by providing individualized care that promotes healthy and independent family functioning. We believe that children and their families are remarkably resilient growth and development when provided with community-centered service and support.
