The HAP Foundation, formerly the JourneyCare Foundation, continues the legacy of expert hospice and palliative care through education and research. The JourneyCare Foundation was founded in 1992 to support community-based palliative and hospice care and to never turn anyone away because of inability to pay. The HAP Foundation, with deep community roots, proudly expands upon that focus providing expert hospice and palliative care education and research. The commitment is to positively impact the experience and future of serious illness by empowering individuals to make informed choices.
