Overview

The Health Hub Project NZ (HHPNZ) is a new population healthcare, health education, and population health research initiative for New Zealand. Essentially, HHPNZ a grassroots social enterprise, which is implementing into action NZ Government health policy, the changes outlined in the NZ Health and Disability System Review, and national and international health research findings. The HHPNZ will: (i) Enable medical and other health professionals to work together in transdisciplinary diagnostic practice teams to deliver personalised population healthcare to ensure NZ communities are at the heart of what we do; (ii) Merge primary and secondary care to form a unified population health care and health education delivery system that ensures an integrated longer-term focus; and (iii) Link closely with community to develop a community voice for the delivery of community-based health and health education. The initiative, when fully established, will provide a wide spectrum of healthcare facilities and services encompassing immediate and urgent care through to elder care, mental health care, maternity services, chronic illness care and support, in-home care support, etc.
