Evergreen Hospice Care is a local, not-for-profit hospice serving the Kansas counties of Johnson, Wyandotte, Leavenworth, Douglas, Miami, and Franklin. We approach care with a conviction that each patient and family should be treated with respect. Individualized care is given based on the goals and wishes of our patients and their caregivers. Our mission is provide person- and family-centered care for mind, body, and spirit at the end of life's journey. Our vision is to enhance the quality of life for our patients and their families. A person-centered approach means to focus on the elements of care and support that matter most to our patient, their family and their caregivers.
