Community Health Services of Union County, Inc.'s mission is to provide personalized, high-quality care to Union County and Anson County residents living with diabetes, high blood pressure, and other chronic diseases. We also accept patients who are needing a primary care physician and those patients who may need wellness exams, and physicals. We have a group of dedicated and experienced physicians, nurse practitioners, physician assistants, nurses and diabetes educators who believe in working with our patients to maintain and improve their health. We work together to provide: Basic healthcare Health screening Community Education Diabetes management Community experiences for students in health professions
