Chronic Care Solutions, L.L.C. provides turnkey chronic care management services for medical practices with patients who have multiple chronic conditions. Chronic care management is a new service model of care developed by Medicare to improve the coordination of healthcare services between patients, physicians, medical facilities, and offices. The ultimate goal of the program is to prevent patient ER visits. We use a unique AI technology to work with practices throughout the United States to provide patients with monthly check-ins that are conducted by our clinical team.
