  Employed by Claremont Living in partnership with Birmingham City Council and University Hospitals Birmingham (UHB). The role of a Homeless Pathway Officer consists of a plethora of responsibilities. Situated in the complex discharge hubs our role entails liaising with housing providers (HMO), ward staff and medical professionals, the psychiatric liaison team and social workers to ensure a safe discharge for homeless patients by sourcing appropriate accommodation. With the main aim to prevent the cycle of homelessness by linking patients to the appropriate services which is achieved through thorough risk assessments to determine the patients capacity, mental health and physical health alongside many other issues.
