HQI serves as a bridge organization for clinical delivery sites to screen patients for social determinants of health and link them to community resources, as well as navigation. ● Project Director for CMS grant, Transforming Clinical Practice Initiative (TCPI) Practice Transformation Network, assisting more than 1,500 primary care providers in Virginia, Maryland, Washington D.C. and West Virginia to be prepared for value based payment. Results for PTN included improving health outcomes for 1,129,772 patients with 95% of practices improving on quality measures; avoiding unnecessary hospitalizations by 20,182; avoiding unnecessary tests and procedures by 12,631; creating a cost savings of $44M. Implementing evidence-based practical solutions for practices to improve quality measures and health outcomes, including producing toolkits and interventions for measures such as clinical depression screening, controlling high blood pressure, diabetes A1c control, annual wellness visits, chronic care management, and transitions of care management. Transitioning practices to value-based payment arrangements, including both up-side and down-side risk. Focus on patient-family engagement strategies, including medication management, use of e-tools, shared decision making, patient experience and patient activation.
