• Extract data from the electronic health record (EHR), eClinicalWorks, to assess and monitor the health of the patient population using IBM Cognos, Query Studio, and Report Studio. • Analyze and present clinical quality, operational, and financial data to leadership to support data driven decision making. • Create and visualize monthly progress reports of clinical quality measures in easy-to-understand formats for the health center’s Board of Directors. • Develop and prepare reports required for NCQA Patient Centered Medical Home (PMCH) recognition and ensure adherence to standards to earn the highest level of recognition. • Update and train staff on required tasks to maintain PCMH recognition. • Communicate UDS, HEDIS, and PMCH measure changes to quality and HIT staff to ensure workflows and EHR documentation continue to align with the measure standards and reporting requirements. • Co-Administrator for eClinicalWorks; collaborate with clinical and operational front-line staff to optimize documentation workflows to improve efficiency and improve staff experience with EHR. • Manage EHR mappings to ensure data accuracy and completeness of clinical measures for annual UDS reporting. • Train clinical staff on proper data entry techniques and workflows to improve data quality which positively impacts care team’s ability to manage their patient population. • Work jointly with quality team to ensure that the Federally Qualified Health Center stays up to date on tasks that is required for federal funding. • Collaborate with quality team and provide data that is used to identify areas in need of improvement, benchmark performance, and measure impact of interventions. • Support practice transformation efforts by building data transparency and creating data sharing practices throughout the organization. Worked under SAHMSA’s SBIRT grant as Greater Danbury Community Health Center’s Health Educator. In this position I incorporated screening for all types of substance use with brief, tailored feedback and advice by utilizing motivational interviewing and a deep understanding of the patients hardships. SBIRT is a comprehensive, integrated, public health approach that provides opportunities for early intervention before more severe consequences occur.
