Whereby I work with the young person and milieu team in establishing and identifying specific ongoing needs for the young persons with focus on their treatment progression, ensuring all of the young persons needs are met where possible. • I am responsible for collecting and recording data on the young person’s progress and presenting it to milieu team on a fort-nightly bases and subsequently implementing any necessary changes to their plan following the multi-disciplinary meeting. • I liaise in a professional and positive manner between direct family members, foster family and external agencies relating to each young person ensuring positive relationship building is maintained and all resources are been effectively implemented for the care of the young person.
