Behavioral Health Home Program was established to improve experience in care, health outcomes, and reduce health care costs and emergency room visits to those with serious and persistent mental illness, chronic illness and on Medicaid. This is accomplished by providing comprehensive care management, care coordination, health promotion, transitional care, patient and family support, and providing referrals to community support services. In this position I was able to provide education and support for 35 clients to gain control over their physical health. Successes include improving client’s nutritional status, gaining access to PCPs and specialists, obtaining health insurance and entitlements, and improving health literacy. I coached clients to be able to live a more independent, healthy life. As a Behavioral Health Home Specialist, I was able to gain experience in supervising/mentoring 2-3 peer case managers, as well as Bachelor level interns. I was able organize and present information to different departments in order for our program to remain successful. Show less

Social Rehabilitation Case Manager

In the social rehabilitation department, I was able to use my creative skills to design and implement educational curriculums, which were used in group settings. This position allowed me to gain first-hand experience in advocating for client services at the Legislative Office Building in Hartford, CT. as well as coordinate luncheons with state and local officials. These experiences taught me how to communicate proficiently with our leaders. While working with the clients I planned monthly meals for up to 25 people, and local and out of state trips, in order for our clients to gain new experiences. This has been most rewarding for me, in my career, since some of the clients would never be able to experience these opportunities. Show less
