      • Full time clinical experience in the home health setting• Rotation mainly saw post CABG, joint replacements, and post fall patients• A typical day included seeing one patient an hour in their home or assisted living facility, and focusing on gaining function within their environment they lived in. Activities such as bed mobility, ambulation, and functional lower extremity strength were worked on. Unique opportunities experienced on this rotation included participating in bi-weekly team meetings with OTs, SLPs, and nurses to report on patients progress. This was also a unique experience not many students get to experience, where I had practice being the only provider around the patient, and was able to work on communication through phone calls, emails, and other forms of communication to make sure every provider was on the same page with each patient. A big part where this was important was if I came into contact with a patient with unstable vitals, and making sure I reported to the doctor when this was found.

Unique opportunities experienced on this rotation included participating in weekly team meetings with OTs, SLPs, nurses, and case managers to report on patients and determine discharge details after leaving Encompass. Was also able to lead family trainings for patients, where an individual family member visited the patient toward the end of their stay, and I was able to answer any questions the family member had about the patient to ensure as safe a discharge as possible.
