A part of the housing and homeless services department, I was a peer specialist on a pilot program called mobile, medical, and mental health team (M3). This was a collaboration with Community Care, Integral Care, and Dell Medical School to provide intensive case management to those with tri-morbid diagnoses. All patients had more than one chronic illness, a mental health diagnosis, and were experiencing homelessness. I provided brief counseling in non traditional settings such as homeless camps. I represented the program at local community events in order to reach a broader population. Held educational classes in diverse settings on HIV/STI harm reduction, and safe substance use.
