I focused on issues of adjustment to illness, support for veterans and families, and linkage to resources to assist with maintaining safety in the community. I supported patients in connecting to state, federal, and VA benefits. I utilized motivational interviewing, relationship building skills, and creative approaches to problem solving in order to assist patients in navigating both the hospital and community systems in an efficient manner. Patients often had co-occurring mental health and health conditions including: PTSD, dementia, Alzheimer’s, diabetes, CHF, COPD, borderline personality disorder, and schizophrenia. As such, I assisted these individuals and families to discover and utilize appropriate social and community resources and connected them to additional care providers to assist them in increasing their interpersonal communication and coping skills. Many veterans and their families also had to contend with homelessness, alcohol and other substance abuse issues, and the long lasting ramifications of military sexual trauma (MST). To assist them in circumventing these barriers, I collaborated with both VA and local community agencies to assist patients in securing appropriate housing, drug and alcohol treatment, and appropriate counseling services.
