I also did data entry and made referrals to the Social Work Department; obtained medical records by date of first appointment. I also provided Information & Referral services and carried the Social Work pager 1-day a week. I did data entry and was the primary care Social Work contact for: Adult Medicine, Pediatrics, Urgent Care, OB/GYN, and WIC & Health Education Dept.
