Assess the spirituality/bereavement health of a patient while identifying strengths & barriers, and while creating an assessment tone and tenor which seems like a conversation. Set goals for support from information gathered during assessment and subsequent visits, then write and update applicable portions of the patient Plan of Care. Translate the effect a patient’s spiritual life/religious background will have on their treatment to the comprehensive interdisciplinary team, resulting in the successful support of patients practicing Hindu, patients who self-identified as spiritual but not religious, and bereaved families practicing Wiccan. Made my availability to prepare and provide religious worship services known to patients, facilities, and the community resulting in musical worship services, Easter communion, sermons on grief and loss, & an ecumenical remembrance worship during the winter holiday season. Officiate at religious and non-religious funerals & memorial services, including deceased patients, deceased persons from partner facilities, & a deceased service dog.
