This division of Michigan Medicine includes Home Nursing, Home Infusion a custom motorized wheelchair manufacturer. We divested our Durable Medical Equipment division in September 2018. This group is working to improve care delivered in the home and help Michigan Medicine improve performance in hospital admission/readmission rates and other population health goals. POM ACO is a Medicare Shared Savings ACO. It is the collaboration of 10 different Physician Organizations across the State of Michigan, comprised of more than 4,000 physicians and serving more than 100,000 patients. As the Executive Medical Director of the East Division, I was responsible for clinical coordination and leading the Clinical Oversight Committee. I served Ex Officio to the Board of Directors as well. POM ACO is a Medicare Shared Savings ACO. The University of Michigan Pioneer ACO (Accountable Care Organization) program is a collaboration between the University… Show more Within the UMHS, the Population Health Office is also responsible for enhancing the clinical performance and coordination of care to optimize our organizational efficiency and manage our costs of care. Our approach to this project creates collaborations across traditional departmental boundaries to enhance the coordination of care of all patients regardless of payer. The University of Michigan Pioneer ACO (Accountable Care Organization) program is a collaboration between the University of Michigan Health System (UMHS), the University of Michigan Faculty Group Practice (FGP), Integrated Health Associates (IHA) and Saint Joseph Mercy Hospital Ann Arbor. This effort ended in 2013. The Population Health Office is charged with managing the administrative aspects of participation in the Pioneer ACO demonstration with Medicare, data analysis of the population metrics and communication within UMHS and across all organizations involved in the ACO. Since the Pioneer project ended in 2013, the Population Health Office serves as the coordinating center for the work of POM ACO in the Washtenaw County area. We continue managing the collaboration between the University of Michigan, IHA and now also Huron Valley Physicians Association (HVPA).

The other 50% of my time was spent with administrative responsibilities. On an organizational level, I served on the University of Michigan Faculty Group Practice (FGP) wRVU Committee setting department reimbursement policy and reporting to FGP senior leadership. The Emergency Department recently moved to a new electronic medical record system (EPIC). I served as a subject matter expert and assisted in the customization of the software, workflow design, program validation testing, trainer training and super-user. I led a multidisciplinary team of doctors, nurses and social workers improving the care we provide to complex and difficult patients. We are working on Health System-wide solutions to decrease recidivism and improve the timeliness and quality of care for this group of patients. Our efforts have resulted in a 75% decrease in ED and hospital utilization for this patient group. The patients involved are highly satisfied with the program and the ED and inpatient staffs are also very satisfied with the impact on patient flow. I Co-Directed the Healthcare Administration Track which is a 20-month, longitudinal curriculum offered to Resident physicians throughout the UM Health System. We provide a foundation of knowledge in Healthcare Administration for those interested in eventually moving into an administrative career. I worked on the ED Lean Team. We use Lean Principles to improve the process by which we treat patients in the ED. We decreased door-to-room time by 25% and improved room-to-doctor time by 5 minutes through our efforts so far. I served on the ED Finance and Billing committees. I have been involved in several QI initiatives.
