      I have worked in various roles as an RN at Saline Memorial. Initially, I started as a Float pool RN. This required me to be knowledgeable in every area including ER, ICU, Med-surg, Ortho, Rehab and our Geri-psych-Adult-psych units. I provided direct patient care in every setting, administering meds, working as a team or individually depending on the unit needs. I dd this for approx. 5 years before joining our Home Health facility where I provided direct patient care in the home setting. This involved IV administration, wound care/ assessment including use of wound vac therapy and educating patients and their families on the importance of compliance and med management. Reinforcing the importance of daily weights, diet restrictions with CHF, Diabetes, CAD, COPD etc to help manage these needs at home. Communicating with their PCP's to help avoid frequent rehospitalizations. This led to my current positon at Saline as an RN case manager on our med-surg floor. I have been in this role for over 3 years. As a case manager i have many responsibilities including UR review, proficient in Interqual guidelines to support the patients clinical needs and am very involved in the DC planning of our patients. Communicating with our Physicians,Therapy team, Respiratory, Pharmacy etc in an Interdisciplinary manner to ensure we are meeting the needs of our patients and their families. This includes facilitating discharges to Acute rehabs, Skilles nursing facilities, or home with home health, obtain DME, including Home Oxygen etc. I am very knowledgable on the Medicare/medicaid guidelines/requirements to help facilitate the dc plan in a timely manner. I love being a case manager as it allows me to be a true patient advocate. Helping patients and their families with their needs outside of the hospital setting. I still occasionally work as a floor nurse, as I never want to lose my skills. Being multi-trained in every area allows me to be the best RN I can be. Show less
