As the Director of Community Health Development with the North Colorado Health Alliance, I engage in the creation of programs that will provide community members of Weld and Larimer Counties with Access and Education in regards to physical and mental health. I oversee 4 programs, offering a wide variety of opportunities for community members of all ages. My Health Connections: An Enrollment Center where certified Health Coverage Guides assist individuals with navigating the difficult task of applying for health insurance. Access to care is the first step to living a better life our HCG’s will assist individuals at No-Cost in the process of securing coverage that includes marketplace carriers, Health First Colorado or CHP+. Healthy Schools Work: Our dedicated and passionate Wellness Coordinators work with 5 Weld County School Districts to assist with and ensure that each school within the districts are offering and incorporating health and wellness into the daily routine of students and staff. Regional Health Connector: Our RHC works to bridge the gaps between providers, patients, and the community. Creating networks of information and services to serve Weld and Larimer Counties. Thriving Weld and Thriving Colorado Data Dashboards: The Thriving Weld Dashboard is a collaborative effort to facilitate data sharing for collective impact in Weld County. Over 70 partners from a variety of sectors use the dashboard to help Weld County bring about measurable and long-term results. The dashboard provides an online gateway to information about how our community is doing in the areas of access to care, active living, education, healthy eating, healthy mind and spirit, livelihood, and health equity.
