Qualified Intellectual Disabilities Professional providing service facilitation for Illinois Home Based Service and Supports Waiver. Case Management.Facilitates the planning, development and implementation of person-centered plans; develops individuals goals and outcomes with people supported; coordinating and participating in individual plan meetings; monitors, documents and reports progress toward meeting outcomes; ensures supports needed are in place to meet outcomes; coordinates communications with other services to ensure individual outcomes are met. Directs and coordinates the activities of all program, treatment and teaching of individuals in assigned homes/locations. Oversees the development of integrated and coordinated active treatment program for each individual on their caseload. Oversees/develops the Quality Assurance plans to monitor the implementation of each program to ensure active treatment. Oversees/provides staff training and supervision to ensure that staff provide quality active treatment and personal cares according to regulations and BQM policy following established routines through coaching/mentoring/training. Ensures timely completion of QIDP responsibilities and paperwork. Ensures contact with families and guardians on pertinent matters and documentation thereof. Provides effective and efficient scheduling of staff to meet the needs and goals of individuals supported and provides on-call 24 hour staff assistance during emergencies.
