      Main duties/responsibilities:• To teach learners with identified SEN in small group setting and on individual basis• To implement, monitor and evaluate provision for individual learners (identified as having SEN) and learner groups, and contribute to school based planning and provision mapping • Usage of ‘Assess, Plan, Do, Review’ model to ensure progress of learners and to document intervention provision• Planning and delivering lessons that are interdisciplinary and integrate technology• Involved in a pilot project, team teaching a year 7 English class, which consists of learners with prior low attainment, in order to provide specialist Literacy teaching to enhance progression. The year 7 class includes 25 learners, of which 8 have a statement of SEN.• Carrying out diagnostic assessments for learners in school (WRATS)• Collating information from Statements/EHC plans, pupils, parents/carers and teachers and creating ‘Pupil Profiles’ in accordance with Pupil Centred approach as highlighted in the new SEN Code of Practice (2015)• Working as part of a team to deliver the best intervention for students (Liaising with departmental and school staff, outside agencies, parents/carers)• Facilitating and being a part of person centred, statutory annual review meetings• Facilitating the successful transition of pupils moving to high school from primary, collating information and ensuring that it is documented accordingly• Visiting primary schools and organising transition days at the high school
