      I work on a social prescribing scheme funded by Somerset Clinical Commissioning Group designed to improve the lives of people who have long-term health conditions or who feel socially isolated. GPs or other health professionals refer patients to me and I find ways to make their lives better. This may be by making sure they are receiving all of the benefits they are entitled to, helping them access social care or equipment to keep them safe and independent in their home, sourcing grants or helping them attend activities they may enjoy. I work closely with other professionals working in health and social care in Somerset and often signpost or refer clients to statutory or voluntary organisations who can provide additional support. I am part of Community Council for Somerset's network of Village and Community Agents. It is a busy, challenging and often incredibly rewarding role. Show less
