- Provided technical assistance and planning support at the local and regional levels in order to advance the health and well-being of rural communities, a chronically underserved population.- Served as the interim Program Coordinator for the Naturally Occurring Retirement Communities (NORC) program until a full-time person was hired.- Under the Southern Tier Population Health Improvement Program (PHIP), a grant funded program which ended in January 2020, I worked with other Population Health Coordinators in Southern Tier PHIP counties to efficiently and effectively implement the NYS work plan and to help address health disparities. As the Coordinator for Delaware and Tioga Counties, I was involved in identifying, sharing, disseminating and helping implement best practices and strategies to promote population health and reduce health care disparities through stakeholder collaboration, data-driven prioritization, and regional or local strategies for addressing health disparities.Successes under this program include data collection and support for the Delaware County Community Health Assessment and Community Health Improvement Plan, coordinating two Blueprint for Health Equity events in which health and human service providers were able to engage with the concepts of equity and social determinants of health, and convening stakeholders around workplace wellness in the form of a regional work group and Wellness Summit, which connected local organizations to free or low cost resources to build their employee wellness program.
