      In this role I visit various aged care facilities and provide them with the following services: 1. Conduct medication chart reconciliation audits against their DAA providers. 2. Conduct clinical medication reviews or RMMR (Residential Medication Management Review) of current medication regimens. This consists of a critical clinical analysis of medication management utilising the most recent and clinically sound evidence. These reviews include liaisons with care staff, as well as medical practitioners, the residents, and their families. 3. Partake in regular Medication Advisory Committee (MAC) meetings where all agenda items are brought forward related to clinical management within the aged care facility, and decision making, as well as policy-making, may take place. Benchmarking data and trends are presented and decisions are made to ensure the ongoing compliance of the facility. 4. Conduct inservices for RNs and care staff, providing education on disease states and medication management relevant to the aged care sector.

      AACP accredited for conducting Home Medication Review consultant. In this role, I have set up an independent business model which requires me to initiate visiting General Practitioners and Practice Managers to educate them on the benefit of HMRs for their patients and thus obtain referrals. Often liaising with GPs to ensure that each review is tailored to them, in order to assist in creating a report that provides the best medication management needs for their patients. This role requires constant continued education in a broad range of areas, but particularly for those associated with managing polypharmacy, deprescribing, pain management, diabetes management, cardiovascular disease state management, and respiratory disease state management. What is involved in the HMR: 1. Thorough history-taking within the patient's home. This covers an understanding of the way they live, and the way this ties into how they manage and understand their medications. Create an understanding of the goals of care from the patient themselves. 2. Identify potential medication-related concerns such as adverse effects or potential interactions (such as drug-drug or drug-disease). This often involves referring back to recent pathology to assist in identifying any concerns. 3. Ensuring that I have ongoing, sound evidence-based knowledge of disease state management to critically think and analyse solutions that may be beneficial to the patient's potentially identified concerns 4. Skills in report writing to assist in creating a medication management plan for the referring GP which aims to assists in providing the best care for their patient's needs.
