The Fund built a health center with one-stop services to include primary care, urgent care, radiology, lab, vision, dental, and pharmacy. My role was to provide leadership to the health center focusing on the development of a population health delivery model. During this tenure, I learned in-depth knowledge on restructuring primary health care services transitioning from volume-focused care to outcome-based care with a key focus on improving health status and quality. I planned and implemented strategic structural changes, delivered precise as well as engaging direction to teams, coordinated with corporate leaders, and led a post-Covid budget. Here are my key accomplishments for this role: • Restored center after COVID and decreased cost by $20M. • Launched EMR and Population Health Tool with a key focus on improving population health outcomes. • Outlined and established the health center's framework to transform from volume-based care to deliver essential value-based care and excellent services. This clinically integrated network allowed the local independent providers that were not ready to be employed by the health system to participate in the development of the population health delivery model and ultimately the value-based contracts we planned to sign. During this period, I acquired comprehensive knowledge on developing a clinically integrated network to amalgamate a network of independent physicians with employed providers in Asante Physicians Partners. The CIN team effectively devised end-to-end population health activities for inpatient, outpatient, community, and state-wide population health entities to accomplish the organization's targeted objectives. I monitored and lead value-based performance metrics among payors with a focus on accelerating value and future cash flow.
