I spent 6 weeks working in long-term care and 6 weeks in subacute rehabilitation. During this time, I gained hands on experience working with various diagnoses often seen in the elderly population. Along with this, I completed morning ADL routines, evaluations, daily and weekly electronic documentation, discharge planning meetings, and interdisciplinary communication to meet the needs of the clients.
