I participate in a specialist palliative care consult service for patients diagnosed with a life-limiting condition. Duties include reviewing and prioritising referrals made to our service. I conduct comprehensive assessments of appropriately referred patients to include physical, psychosocial, and spiritual needs. I provide advice regarding symptom management, assistance with future care and discharge planning, aid with the management of ethical dilemmas and end-of-life care. I participate in the development and implementation of plans in conjunction the multi-disciplinary team, the patient and their family as appropriate. These plans are monitored, evaluated and amended accordingly. I provide support, information and education to staff, patients and their families as required. I participate in medication reconciliation taking cognisance of poly-pharmacy and support medical and pharmacy staff with medication reviews and medication management. I am a strong patient advocate and maintain respect, dignity and confidentiality for the patient and their family at all times. I facilitate education sessions for all hospital staff in relation to evidence-based palliative care delivery (e.g. Breaking Bad News, Final Journeys and other informal education sessions). I provide support for all staff, in particular, nursing staff and junior medical staff in relation to palliative care. I identify, initiate and conduct audit relevant to the area of palliative care to identify needs of patients and staff and improve service provision. I contribute to the development and implementation of clinical standards and guidelines taking into consideration policies, protocols and guidelines.
