  The Dutch maternity care system is divided into midwife-led and obstetrician-led care. Women with low risk pregnancies receive midwife-led care from community midwives and have a choice to give birth either at home, in a birth centre, or in a hospital with their community midwife. Women with risk factors or complications in pregnancy or during labour are referred to a hospital where they receive obstetrician-led care from a team of hospital-based midwives, obstetric registrars and obstetricians. In 2020, 50,5% of women who gave birth for the frst time in the Netherlands started labour in midwife-led care, and 16,7% gave birth assisted solely by their primary care midwife. For multiparous women these numbers were 47,8% and 34,3%. Primary care midwifes work as independent healthcare professionals and make, together with women, autonomous decisions about childbirth (interventions) or referral to obstetrician-led care. At GCM we are a small team, ensuring 24/7 availability of a known midwife close to home and in the home of the client, in collaboration with medical and allied health care practitioners. We provide evidence based care for women with low and slightly raised risk pregnancies during pregnancy, labour, birth and the postpartum period.
