I worked with people with reduced balance and/or strength requiring reconditioning to enable them to return to previous levels of mobility. This involved working for the Rehabilitation and Falls Unit in conjunction with physiotherapists and occupational therapists on the Rehabilitation Ward and in the community. I worked for the rapid assessment team; we prevented hospital admissions and facilitated hospital discharges. We provided assistance for up to 6 weeks, up to 3 times a day. We assisted with people who had been in hospital for long periods of time and I helped them adapt to reduced levels of mobility and to make adjustments to their daily activities. We provided mobility and exercise visits. I supported individuals who, whilst requiring hospitalisation, were reluctant to be admitted supporting them until their recovery or until the necessary care had been put in place.

I was a care assistant in the community which involved assisting people with personal care, medication prompts, meal preparation and visits to the home to carry out a range of duties. I was involved in risk assessments as the company took on new clients, assessing their home environment and making suggestions on improving safety for themselves and caring staff. Regular reviews were carried out every year or as circumstances changed.
