      Working within the Child Health and Disability Team, this role has been similar to my previous Social Work position for Somerset County Council, however it has allowed me to further expand my knowledge and experience in relation to all Child Protection matters and Safe Guarding concerns. The main responsibilities of my role being: • Case managing between 20 -25 families at a time. • Completing Initial and core assessment of children and families in order to recommend eligibility and prepare appropriate Government service schedules and associated funding. • Application for end-to-end facilitation of services for children with disabilities and associated family. • Initiation of child protection conferences and management of child protection cases. • Review, optimisation and development of existing care packages to meet evolving needs of those under care. • Completion of court chronologies for children with disabilities and children at risk of significant harm. • Completion of ‘Looked after Child Processes’ safeguard process. • Case by case documentation and administration. • Risk assessments for children with disabilities and carers. • Complex, sensitive problem solving. • Budget management and commercial responsibility for services offered/delivered. • Delivering presentations to various professionals with regards to all of the above in order to ensure children are safe from harm and to ensure that they receive appropriate services.
