Achievements to date: • Launched Central Bedfordshire SP Service in 11 GP surgeries across 4 locality areas, generating 130 complex and multiple need referrals in approximately 8-week period • Launched Bedford Borough SP Service in 7 GP surgeries with limited resources and a smaller delivery team. Secured new SP contracts with 2 self-funded GP cluster groups to deliver their internal SP service due to robust service design and support and management systems • Secured NHS email account for single-point-of-access; rare occurrence for a charity and an external NHS provider • Have over-performed on Local Community Coordination contractual cost savings in Q2 & Q3 since taking over project • Increased referrals and target cohort, improved client outcomes by introducing a health coaching approach, and widened reach of service Show less

Brought in to put Project Plan back on track and to develop and manage the service: rapidly determined 10 week delay, no targeted client base identified, and no referral pathways • Negotiated and managed delivery requirements for Service Level Agreements with Police and Probation, set up a Single Point of Access (SPA) for referrals into the project, set up monitoring systems to record all aspects of project activity including actions, outcomes, referrals, and the achievement of targets, reported on service and partnership performance • Provided day-to-day management, support and guidance to project workers and volunteer peers, allocated work, trained staff, organised the promotion of the project, coordinated the referral and engagement of service users, developed support services across 9 pathways out of crime, ensured information and advice services were seamless and aligned for service users, and set up a comprehensive programme of workshops • Exceeded project referral and outcome targets: improved employability skills, mental wellbeing, physical health, reduced social isolation, and achieved behavioural and lifestyle changes for 240 women.
