Implementation of a new healthcare program called Hospital in the Home (HITH) on the Sunshine Coast in 2014, funded by the Queensland Government and offered by the SCHHS. HITH involves the provision of multidisciplinary acute care to suitable, consenting patients, at their usual place of residence as a 100% substitute for inpatient hospital care. HITH National and International reviews have prompted the increased use of HITH as a model that gives patients greater choice in their care; improves access to health services; provides equal or better outcomes and improves efficiencies in service delivery. Quality Initiatives: The SCHHS model of HITH is an innovative 1st off model in Qld and in some aspects a 1st off in Australia. HITH patients are cared for with External or Internal Medical Governance, including having the option of clinical governance done by a Nurse Practitioner. State-wide key performance indicators are used to measure and monitor the success of the programme and identify areas of improvement. HITH has been implemented district wide in the Nambour, Caloundra, Gympie and Maleny Public Hospitals and for public patients in the Sunshine Coast University Private Hospital, as well as setting up a Direct Referral process for primary care to refer directly into HITH. All policies, procedures, clinical guidelines and HITH processes have been developed since commencing the role in May 2014 to meet ACHS standards of accreditation. Show less

Principal Project Manager, Project PalliAPP - Palliative Admission Prevention Programme

The clinical problem that initiated the project was that SCHHS staff identified a gap in services regarding avoidable admissions for palliative care patients. Amidst the restructuring of service delivery models and an emphasis on reducing avoidable admissions and presentations, solutions to providing a more patient-centred model of care was a priority of the SCHHS. The absence of a targeted initiative aimed at avoiding admissions gave impetus to the establishment of “Project PalliAPP”. To date I have made substantial wins and gains by the engagement, partnership and links with stakeholders on the project to improve support in the community and reduce admissions.Quality Initiatives: developing resources for patients and carers to reduce carer stress, including a magnetic fridge magnet with contacts. Commencing a General Practitioner engagement programme with the Palliative Care Service, improving community collaboration and increasing GP education. Improving and increasing collaboration and engagement with the Residential Aged Care Facilities on the Sunshine Coast, including the Palliative Approach Toolkit Rollout, November 2013. A palliative care education programme with the Intern education programme was commenced in the SCHHS in November 2013. An education programme to increase Advance Care planning education and raising awareness to improve patient care wishes for end of life care decision making. Improving discharge planning and clinical handover, by ensuring 100% of enterprise discharge summaries and medication discharge lists accompany all palliative patients on discharge. End of Life portfolios to be established in all workplaces in the SCHHS to align with ACHS Standard 12 End of Life Care standards in accreditation. Show less
