Wraparound is a process by which members of a child’s treatment team come together regularly with the child and family to plan for how to best meet goals and needs through providing skills training through therapeutic intervention and education. The child and family are empowered to choose who will be on the team, often comprised of family members, friends, natural supports, and community service providers, creating a plan that reflects the family’s culture, strengths, needs, and goals. The facilitator monitors and adjusts plans to meet identified training deficits, providing support to both child and family, meeting all state compliance regulations. ● Develops, writes and updates person centered Care Coordination plans, to include employment assistance. ● Documents all services for billing and tracking purposes in compliance with state documentation. ● Coordinates services offered by multiple agencies. ● Collaborates with families and all child serving agencies in the County to coordinate the provision of training and services. ● Collaborates with local treatment providers to coordinate care and transitions to and from this level of care. ● Provides consultation to all child-serving agencies and to the general public about services at all levels. ● Seeks out resources for families other than those provided by the mental health system. ● Helps youth and family strengthen and expand natural and social support systems. ● Provides consultation and training to other community organizations to support process. ● Certifies annually to administer CANS/CAFAS/PECFAS assessments. ● Guides the transition of the youth back into community setting by assisting the family with youth driven goals working toward self-sufficiency. ● Guides the engagement process by exploring and assessing strengths and needs in order to meet goals. ● Guides crisis plan development and provides safety planning, monitors the implementation and may intervene during a crisis during day hours or on-call.
