Every three months an Individualized service plan is made with the case manager, youth and myself to track the client’s progression. Managed a case load of 5 adult clients who experienced psychosis in a group home setting. Each client provided personal goals for themselves and workers provided support in achieving those goals. Some goals would be finding housing, going back to school, attending therapy groups and so on. Weekly house meetings were run, and chores were assigned to each resident.
