Achieve Australia is a company limited by guarantee, governed by a voluntary Board of Directors. They support approximately 550 people with disability or who experience disadvantage across accommodation, employment and day programs on the basis of achieving social inclusion. The driving principle is to support people with disability to learn and maintain life and work skills, develop relationships, make choices about their lives and participate in community activities. The Team Leader position reports to The Chief Operating Officer and is responsible for 4 staff with approximately 250 total staff in organisation.•Overseeing and guiding a team to provide specialist services throughout the organisation•Assisting with the management of the intakes and exits of people for the Accommodation and Day Program services•Assisting with the transitions of clients within Accommodation services•Liaising with external stakeholders and families regarding services and intake information•Keeping up to date with knowledge of individual funding packages and guidelines•Participating as a member on a restricted practice panel to ensure the organisation meets ADHC policies and guidelines•Reporting to the committee on all intake/exit transition cases.
