The HOA program is a new initiative for SSSEVA. The program is in equal partnership with The Planning Council, the lead agency for the southeastern Virginia CoC. Homeless older adults are in a unique place where housing, health care, and aging intersect. The HOA program improves services and supports for homeless older adults in the region at both the systems level and the individual level. The program achieves this by co-leading the working Coalition for Homeless Elders and facilitating SSSEVA services for homeless older adults. With the HOA program SSEVA's mission is realized: To provide seniors and their caregivers with access to programs and services so they may live their lives with choice and dignity in their communities.• Co-leader of the Coalition for Homeless Elders. -A working coalition of diverse stakeholders from the aging, homeless, and health care sectors in southeastern Virginia. The collaborative work of the coalition on specific initiatives results in the improvement of systems and services for homeless older adults in southeastern Virginia. A few outcomes of the coalition's work are increased housing stability, positive health outcomes, and the avoidance of unnecessary institutionalized care for these individuals. The Coalition for Homeless Elders is committed to understanding the unique needs of homeless older adults and understanding each other to serve this population as one.• Facilitator of SSSEVA services and supports for homeless and formerly homeless older adults in PSH and other locations. These services increase housing stability, aging in place, and quality of life for these individuals by supporting independence, dignity, maintaining current abilities, and the aging process.
