Senior member of the East and North Hertfordshire Care Home Pharmacy Team (1 of 6 national Vanguard sites for ‘Enhanced health in Care homes’) – which is leading the way forward with personalised and outcome-focused care, by optimising medications and reducing Polypharmacy. Working directly with care home management teams and patients to help improve care and lives of care home residents as well as contributing to driving strategic change to make significant improvements in care across the NHS and UK for elderly care home patients. She has been a strong advocate for this initiative and has been interviewed by the Pharmaceutical Press, quoted in articles and also interviewed by BBC Radio 4. Responsibilities:  Championing the use of technology to improving healthcare for care home residents.  Selected for piloting NHS Digital using skype in care home consultancies.  Working closely with care homes and GPs to reduce Polypharmacy by level 3 clinical medication reviews.  Optimising medications, deliver better outcomes, lifestyle and wellbeing of patients, especially in high risk patients.  Proposing alternatives to reduced medicine waste and reduce costs for care homes and the wider NHS.  Provide education and training on medicines optimisation in care homes via face-to-face and webinars for clinicians and care home staff.  Ensuring compliance with relevant medicines legislation and support CQC actions.  Supporting the safe and effective transfer of patients between different NHS organisations.  Supported on the development of ‘Good Practice Guidance’ for care homes.
