• To deliver the highest standard of Dialysis treatment to the patients, to provide continuous health education and to establish a good professional working relationship with colleagues. • Reviews physician orders for dialysis patients collect pre-treatment dialysis data and reviews patient records before dialysis. • Perform pre-dialysis patient assessment including patient vital signs, dry weight and patency of the vascular access, laboratory findings, and the patient’s mental health. • Perform physical examination including Inspection, Palpation, Percussion and auscultation. • Verifies that patients are taking all prescribed medications and performs medication safety by following eight rights. • Document the information regarding patient assessments and interventions and notify the physician if any significant change in the patient's condition and other pertinent information. • Awareness in digital documentation. Documentation should be clear, concise and accepted abbreviations by TQM. • Discusses patient concerns and answers the questions relevant to care. • Assess, prepares, and cannulates a patient’s vascular access (including needle insertion in arteriovenous fistula or grafts by following aseptic techniques. • Oversees the preparation of the delivery system, dialysate bath, and dialyzer and confirms that all mandatory alarm tests on the dialyzer and equipment. • Apply sterile techniques to assess the dialysis catheter exit site and to apply to dress on the patient’s vascular access per protocols. (including non-tunnelled and tunnelled catheters)Maintains skills necessary to perform surveillance on dialysis access include catheter, fistula, graft for early signs of failure or infection. • Collaborating with the nursing team to ensure CVVHD, CVVHDF, hemoperfusion and plasmapheresis therapy in ICU and CCU. • Vital signs monitoring every hour for stable patients, every 30 minutes for unstable patients and every 15 minutes for critically ill patients.
