Working in a variety of settings, I have developed personal, flexible and realistic support packages based on people's individual circumstances. In this busy role, no two days will be the same as I manage a large, interesting caseload without compromising on quality. I coordinate individual treatment journeys and offer the people-centered care they come to us for. That means building relationships with families and associates to make sure each person gets the support they require. Most of my work is outreach basis across the area including visiting schools, children's family practices, youth clubs and other venues, in order to provide interventions to young people. Duties also include delivering 1-1 and group work interventions, targeted outreach to engage hard to reach young people and delivery of professional workforce development packages to partner agencies. Ultimately, this role is all about helping young people to get back on their feet, so l develop strength-based recovery plans, complete risk assessments and identify opportunities for service users to get involved in their communities. I have the ability to co- ordinate and complete care plans, assessments and strong knowledge of substance misuse issues. Show less
