  Placement- Independently managed a client caseload of people aged 65 years and older, who have had a recent admission to hospital or a medical condition/illness which had resulted in a decline in health- Client caseload included 8 community clients and 8 bed-based clients in TCP and a varied caseload in HACC- Consolidated skills in conducting assessments including cognitive assessments such as the Standardised Mini Mental State Examination (SMMSE), initial assessments and home assessments- Built a professional relationship with community services including Aidacare, community care builders, support groups and outlying bed-based services- Independently completed documentation for care plans, referrals, Service Co-ordination Template Tools, file notes and letters to clients- Evaluated client’s progress through functional reviews, self-reporting, liaising with family members and/or significant others
