      Writer assist Access Hotline (DBH) with linking the consumer for Mental Health Services, Substance Abuse and Counselling Services at HCFC. Consumers only can be linked to HCFC through Access Hotline 1-877-793-4357. Writers other duties included scheduling Adult Intake appointments. Writer adds new consumers to our Credible data base by filling out the demographics potion i.e. address, dob, SSN, ethnicity, phone number, emergency contacts, race, and whom referred them. Writers also checks to see if the consumer has insurance before the intake takes place by checking DC.Medicaid.com data base. Implementing consumers insurance in our data base. Writer informs consumer of the date and time of the appointment and documents that are required i.e. a DC government ID and proof of insurance. Writer also schedules diagnostic assessments for new and old consumers. Scheduling of psychiatric appointments and evaluations and follow ups. Writer schedules the conference rooms on MLK for other entities that are apart of our agency to use. Writer set up luncheons for pharmaceutical reps to present their medications to the Psychiatrist/Nurse Practitioner and how they will affect our consumers if taken. Writer is still a part of the other location at 915 R.I. Ave, NW and helps to coordinate the psychiatrist/nurse practitioner schedule as well. As well as other duties that may arise. Show less

      I assist consumers once linked to HCFC with applying for social services i.e.. SSI/SSDI, food stamps, DC Medicaid. Finding safe and affordable housing i.e. SRO, subside housing programs, transitional housing programs and Oxford Houses by assisting them with filling out the paper work or getting documents needed i.e. police clearance, birth certificate, social security card and funding for security deposit. Made sure that they keep all psychiatrist and medical appointments by attending the appointment with the consumer. Help them coordinate taking their medications (with the assistant of the ACT Nurse) on a daily basis. Help strategies a crisis plan, treatment plan and day program with the consumers assistance. Meet with consumers 2-3 times a week to assist them with their ADL Skills i.e. hygiene( if shower is warranted assist with hygiene products and secure the shower/bathroom for consumer), supply clean clothes if any donations are available for them, help with budgeting their money, buying clothes and groceries, paying bills, helping with admission or discharges from hospitals and or 28 day treatment programs through APPRA.. Did monthly budget for consumers that Bread for the City were their payee. Assist with coordinating/facilitating the Substance Abuse group every Tuesday from 2pm-3pm and Women's Rap on Wednesday from 2pm-3pm. Other duties that were assigned by the Act Team Leader.
