  We are currently implementing a Transitional Care Provider Program I designed in conjunction with the stakeholders within our healthcare system. I provide primary care to individuals who have recently been discharged from the hospital and do not have a primary care provider. I provide primary care to individuals who have recently been discharged from the hospital and do not have a primary care provider. A significant portion of individuals who are discharged from the hospital without a primary care provider have difficulty establishing with a primary care provider within 30 days. As a result they fall through the cracks of the healthcare system and many require re-admission to the hospital due to lack of follow up. My goal is to provide hospital follow up within two weeks and provide a bridge to primary care by meeting the immediate medical needs of these individuals over 30-60 days while also working to help them establish with a primary care provider who can take over for the long term.
